Heart Failure (HF) is a chronic progressive disease with less than 50 percent of patients living five years after their initial diagnosis, and less than 25 percent are alive at 10 years. Our current system of HF care, characterized by a cycle of acute hospitalizations and acute episodic outpatient care by a primary or emergency room provider, has not led to great improvement in patient quality of life or prognosis. The Manual of Heart Failure Management provides readers with a concise evidence-based approach to key issues in the evaluation and treatment of heart failure. Important issues in prevention are explained, as well as the state of the art pharmacological and device (VAD) therapies that are available to clinicians. The role of pacemakers and implantable defibrillators are also assessed, and concise clinical guidelines are described. The recommendations within this manual are based around the current recommendations and guidelines for the treatment of heart failure. By uniting each individual management concept into a comprehensive approach to HF, this book outlines the tremendous obstacles facing patients but also the advantages of dealing with the management of HF in a strategic way. Understanding the next step of treatment beyond your own caretaker abilities is critical for patient care and education, for changing public perception, and for translating research opportunities into treatment protocols for the future.

Acute heart failure is a potentially life threatening situation where correct, rapid therapy can have a life saving impact. In this rapidly changing area of medicine this text aims to bring the latest understanding of the pathophysiology together with a practical guide to diagnosis and management using a thoroughly evidenced based approach. This will appeal to a wide audience of health care professionals who will treat patients with acute heart failure, including doctors, medical students, nurses, and other professionals in the Emergency Department, General Internal Medicine, Anaesthetics, Cardiology (Medicine and Surgery) and Critical Care settings. It may be used as an evidence based guide for the junior practitioner or as an aide-memoire for the more senior. Always keeping the underlying pathophysiology at the forefront of the discussion, the reader is encouraged to understand the aetiology of the acute situation and how to direct management in order to correct the abnormal physiology. Each chapter is heavily referenced allowing the reader to easily refer to the original experimental studies and pursue topics in more detail if required. This text is a valuable addition to any practitioner who treats patients with acute heart failure and wants a deeper understanding of the condition.

Both thyroid dysfunction and heart failure show a high prevalence in the adult population. Frequently, in clinical practice, a multidisciplinary approach is useful to optimize the management of patients with these conditions. Although there is no doubt regarding the close link between cardiovascular pathophysiology and thyroid homeostasis, our understanding of this association is far from being exhaustive. Thyroid hormone regulates the expression of cardiac-specific functional contractile and structural proteins and plays a pivotal role in modulating both diastolic and systolic function as well as peripheral vascular resistance. The close relationship between thyroid and heart dysfunction is strongly supported by recent evidence demonstrating that an altered thyroid profile is a negative prognostic predictor in patients with heart failure. The treatment of chronic heart failure, especially in advanced stages of the disease, continues to be an open and challenging field. The potential of novel thyroid hormone therapies that address the molecular biology of thyroid dysfunction and heart failure thus represents an attractive area of multidisciplinary scientific interest. This book is a readable, integrated, and highly up to date presentation of the clinical, pathophysiological, and basic science aspects of thyroid–heart failure interactions. It addresses a complex subject in an approach that targets a large audience of readers.
